Patient Rights

At Lake Roosevelt Community Health Centers, we are committed to upholding and respecting the rights of our patients. We believe that every individual receiving healthcare services has the following rights:

2. Right to Privacy and Confidentiality: Patients have the right to expect that their medical information will be kept confidential and shared only with those involved in their care, unless authorized by the patient or required by law.


3. Right to Informed Consent: Patients have the right to receive clear and understandable information about their diagnosis, treatment options, risks, benefits, and alternatives. They have the right to make informed decisions about their healthcare and to give or withhold consent for any medical procedures or treatments.


4. Right to Access and Continuity of Care: Patients have the right to timely access to healthcare services and to continuity of care. Patient Responsibilities

At Lake Roosevelt Community Health Centers, we believe that patients have certain responsibilities in order to ensure the best possible healthcare experience for themselves and others. We ask our patients to:


1. Provide Accurate Information: Patients are responsible for providing accurate and complete information about their medical history, current health status, medications, and any other relevant information that may impact their care.


2. Follow Treatment Plans: Patients are responsible for following the agreed-upon treatment plans, including taking medications as prescribed, attending scheduled appointments, and following any recommended lifestyle changes or self-care instructions.


3. Ask Questions and Seek Clarification: Patients are encouraged to ask questions, seek clarification, and actively participate in their healthcare decisions. It is important to understand the information provided by healthcare providers and to communicate any concerns or uncertainties.


4. Respect Appointments and Timeliness: Patients are responsible for arriving on time for scheduled appointments and notifying the clinic in advance if they need to cancel or reschedule. This helps us provide timely and efficient care to all patients.
