Patient Responsibilities

At Lake Roosevelt Community Health Centers, we believe that patients have certain responsibilities in order to ensure the best possible healthcare experience for themselves and others. We ask our patients to:

1. Provide Accurate Information: Patients are responsible for providing accurate and complete information about their medical history, current health status, medications, and any other relevant information that may impact their care.

2. Follow Treatment Plans: Patients are responsible for following the agreed-upon treatment plans, including taking medications as prescribed, attending scheduled appointments, and following any recommended lifestyle changes or self-care instructions.

3. Ask Questions and Seek Clarification: Patients are encouraged to ask questions, seek clarification, and actively participate in their healthcare decisions. It is important to understand the information provided by healthcare providers and to communicate any concerns or uncertainties.

4. Respect Appointments and Timeliness: Patients are responsible for arriving on time for scheduled appointments and notifying the clinic in advance if they need to cancel or reschedule. This helps us provide timely and efficient care to all patients.

5. Respect Staff and Other Patients: Patients are expected to treat healthcare staff and other patients with respect, courtesy, and dignity. This includes following any rules or guidelines set forth by the clinic and maintaining a cooperative and considerate attitude.

6. Provide Feedback and Voice Concerns: Patients are encouraged to provide feedback about their healthcare experience and to voice any concerns or complaints. This helps us improve our services and address any issues that may arise.

7. Take Responsibility for Personal Health: Patients are responsible for taking an active role in maintaining their overall health and well-being. This includes adopting healthy lifestyle habits, managing chronic conditions, and seeking preventive care as recommended.

By fulfilling these responsibilities, patients can contribute to a positive and collaborative healthcare environment and help us provide the best possible care to all individuals in our community.
